El projecte Indellar consisteix en implementar un programa social, finan\u00e7at per la UE – Next GenerationUE i la Diputaci\u00f3 de Barcelona, per a la millora de la qualitat de vida i l’autonomia per a persones majors de 65 anys que es troben en situaci\u00f3 de depend\u00e8ncia i\/o complexitat social i vivint soles o amb una persona cuidadora que formi part del col\u00b7lectiu gent gran. El programa est\u00e0 dissenyat per a atendre persones que viuen a Cubelles, Canyelles i Olivella de la comarca del Garraf i volen viure de manera independent, tenint en compte el seu projecte vital combinant la tecnologia amb un enfocament personalitzat, contingut diversificat i suport continuat. En aquesta l\u00ednia, des de l\u2019abril fins a finals d’octubre de 2024 es desenvolupar\u00e0 un pilot per a aplicar aquest programa el qual s’emmarca dins del projecte \u201cGARRAF, COMARCA CUIDADORA\u201d, una iniciativa del Consell Comarcal del Garraf.”<\/div>\n

El projecte IndeLlar<\/b>, t\u00e9 com a objectiu crear un nou model escalable que aporti autonomia, millori l\u2019acompanyament, la qualitat de vida, l\u2019accessibilitat i continu\u00eftat de l’atenci\u00f3, mitjan\u00e7ant una atenci\u00f3 integrada, flexible, on s’evidencia una bona transici\u00f3 entre els diferents sistemes de suport socials, salut, educatius i d’altres \u00e0mbits implicats, en tot el cicle de la vida.<\/p>\n

IndeLlar<\/b> permet obtenir una valoraci\u00f3 objectiva dels resultats en benestar f\u00edsic, cognitiu, emocional i relacional per valorar l\u2019impacte de les pol\u00edtiques d\u2019atenci\u00f3 integrada social i de salut en la millora de la qualitat de vida: m\u00e9s dies, amb m\u00e9s bona salut, a casa, disminuci\u00f3 dels factors de risc, millora de l\u2019autocura i altres.<\/p>\n

L’objectiu \u00e9s que, amb aquest projecte pilot, es promogui un model d\u2019atenci\u00f3 innovadora que es pugui aplicar en el conjunt del sistema i del territori, de forma escalable i assoleixi, aix\u00ed, l\u2019impacte en un \u00e0mbit s\u00f2cio-sanitari amb alta necessitat.<\/p>\n
